    Request for certificates with terms including, but not limited to, any of the following requirements requires that you contact the Agency:
      a.  Written notice beyond 10 days (with the exception of workers compensation which is 20 days).
      b.  Additional Insured.
      c.  Waiver of Subrogation.
      d.  Hold Harmless agreements.

    Fawcett Dopke Agency is not responsible for certificates issued with changes that do not receive prior approval by the Insurance Company.
